BHCOE Forms

BHCOE Request for Audit Rescore
Organizations that the BHCOE has conducted a review of and that have received a Preliminary Accreditation, Full Accreditation, Provisional Accreditation or Non-Accreditation may request the BHCOE to re-score their accreditation review within 60 days of completion of the debrief call.

BHCOE Change of Information Form
Accredited organizations are required to submit this form within 30 days of completion of any material changes. This includes, but is not limited to, change in ownership, change of corporate name, change of address, and addition or closure of sites, as well as any claim, proceeding, or investigation by a government or regulatory entity.
